If you
are considering nominating yourself or another individual,
please complete the following steps to create a
nomination packet. Once completed the nomination packet must be sent to
arrive in the AEA office, either via email or landmail,
by the deadline in order to be considered for a 2012 award.

STEPS
TO SUBMIT A NOMINATION

description and nomination requirements for the specific

5. Compile the required
submission materials either electronically or in hardcopy
depending on your chosen submission format |

SUBMISSION FORMAT
You have two options for submitting your nomination
packet.
A. Email/Electronic Nomination Submissions
- All documentation must be combined into a single electronic file and sent as a single email attachment,
beginning with the Nomination form.
- AEA requests that you compile the materials in the order specified in the
Nomination form.
- The electronic versions of all the required materials should be compiled into one Word or PDF file and
sent as an email attachment. Please do not send an email with multiple attachments.
- Each item in the nomination packet should clearly identify the nominee and the award for which
he/she/they are being
nominated.
- Letters of recommendation that must remain confidential can be sent
electronically to the AEA office at
info@eval.org separately from the nomination
packet, but must be properly noted on the nomination form, and
sent electronically to arrive in the office by the deadline.
- Nomination materials that are not included in the electronic
nomination packet or that arrive in the AEA office after the
submission deadline will not be considered.
- All nominations materials should be emailed as a single file
on or before Friday, June 8, 2012 to
info@eval.org.
B. Landmail/Hardcopy Submissions
Please use this option only
if electronic submission is not an option due to particular parts of
your nomination being only available in hardcopy format, such as the
need to include a book or report. Do not send both electronic and
hardcopy.

The Nominator must compile all nomination materials into a
hardcopy packet and then make 5
copies of the
hardcopy packet to send to the AEA office. (copy and send the materials as
5 complete sets rather than five
copies of each individual document.)

Each item in the nomination packet should clearly
identify the nominee and the award for which they are being
nominated.

We request that you compile the materials in the
order specified in the Nomination form before making your
copies.

Letters of
recommendation that must remain confidential can be sent to the AEA
office separately from the hardcopy nomination packet, but must be properly
noted on the nomination form, and sent to arrive in the office by
the deadline.

Nomination materials that are not included in the hardcopy
nomination packet or that arrive in the AEA office after the
submission deadline will not be considered.

NOMINATION FORM
The completed nomination form acts as a cover page
for the nomination packet as well as an inventory of the nomination
materials being submitted. Only those materials listed on the
nomination form will be considered as part of the nomination packet.

- All required nomination materials including the
completed nomination form have been compiled into a single Word file in the order listed in the nomination form.
- All nominations materials are prepared to be emailed to arrive by Friday, June
8, 2012.
Congratulations! You are ready to submit
your nomination packet!
Please attach your nomination packet to an email
and send to Heidi Nye in the AEA office at
info@eval.org.
B. Submit your nomination packet
via LANDMAIL
Please double-check that:
- You have downloaded and completed the
nomination form.
- All required nomination materials including the
completed nomination form have been compiled into a single packet in the order listed in the nomination form.
- Five hardcopy sets have been made of the complete nomination packet.
- All nomination materials will be mailed via landmail
to
arrive by Friday, June 8, 2012
Congratulations! You are ready to submit
your nomination packet!
